SLGL: Bear thesis: topical patidegib will fail Phase 3 again (readout Nov 2026). First Phase 3 (NCT03703310) failed in 2020; new trial repeats same disease/endpoint with fewer patients (~140 est vs 174), so worse-powered. Core problem is topical delivery: patidegib's physicochemical profile (~505 Da, XlogP 4.6) doesn't favor deep-dermal penetration, and a comparable molecule (vismodegib) showed essentially zero deep-dermal exposure through intact skin without ablative laser assist. Author sees the drug as active but undeliverable to BCC-forming compartments—hence 'zero.' Risk: readout could still surprise, and cross-molecule PK read-across is inferential, not direct patidegib data.
