As of April 1, 2015 Aviv REIT, Inc. merged with Omega Healthcare Investors, Inc. Aviv REIT, based in Chicago, specializes in the ownership and triple-net leasing of post-acute and long-term care skilled nursing facilities (SNFs). We have been in the business of investing in SNFs for over 30 years and own one of the largest portfolios of SNFs in the U.S. Our diversified portfolio consists of 357 properties in 31 states leased to 37 tenants. Aviv has developed a strong track record over 30 years as a reliable and relationship focused provider of capital.
